About
For erectile dysfunction: It is estimated that around 57 million men worldwide experience difficulty in achieving and/or maintaining penile erection. Only about 10% of these men actually receive treatment.
Tadalafil is used to treat erectile dysfunction. It is effective for up to 36 hours after administration, which is a major marketing plus for its manufacturer. They say that this allows for more spontaneity in love making than Viagra which has to be taken between 30 minutes and 4 hours before sexual activity.
It works in a similar manner to Viagra by facilitating increased blood flow to the penis and thus promoting increased hardness and longer maintenance of erection.
As with Viagra, Tadalafil (Cialis) has no effect if there is no sexual stimulation.
For pulmonary hypertension a rare condition with an annual incidence of 1 or 2 per million. It is used in many countries in the treatment of pulmonary hypertension, although it has not yet received FDA approval for such in the
Dosage
For sexual dysfunction: Initially, it is advised to take 10mg at least 30 minutes before sexual activity and to adjust subsequent doses between 5mg and 20mg. These are the dosages for patients who intend to use the drug as and when it is needed.
If it is required to take Tadalafil (Cialis) every day, then a lower dose is recommended - 2.5-5mg daily. In this case, sexual activity can be undertaken at any time inbetween dosages.
For pulmonary hypertension You are advised to seek the advice of your doctor or pharmacist as doses vary.
Avoiding eating grapefruit or drinking grapefruit whilst you are on this medication.
Store in a dry place at room temperature out of the reach of children.
Side Effects
The most common side effects are at least partly due to Tadalafil’s effect of vasodilation. They include headache, indigestion, back pain, muscle aches, flushing, stuffy or running nose. These side effects are generally temporary, disappearing after a few hours.
Sudden hearing loss is another possible side effect, and you should stop taking this medication, and seek medical help if this occurs.
Rarely Tadalafil may cause serious cardiovascular event.
Precautions
Do not take nitrates whilst using this medication, e.g. amyl nitrate (used as a sex enhancing drug), or glyceryl trinitrate (a spray used to alleviate the symptoms of angina).
The British National Formulary lists moderate heart failure, uncontrolled arrhythmias, and uncontrolled hypertension as contraindications to the administration of Tadalafil, or in patients with a previous history of non-arteritic anterior ischaemic optic neuropathy. It also lists hypotension, recent stroke, unstable angina and myocardial infarction.
Tadalafil should also be used with caution if you suffer from cardiovascular disease, anatomical penile deformation, sickle-cell disease, multiple myeloma, or leukaemia.
If your erection lasts over 4 hours you should seek medical help as soon as possible, in order to avoid damage to the sensitive tissues of the penis.
How it works
As for Viagra, Tadalafil is a phosphodiesterase type-5 inhibitor which by its inhibitive properties, causes relaxation of the penile arteries and the corpus cavernosal smooth muscle of the penis. This allows greater blood flow to the penis and thus easier establishment and maintenance of an erection.
It is longer lasting that Viagra, Tadalafil having a half-life of 17.5 hours as opposed to 4 hours for Viagra. As it is effective for about 36 hours, Tadalafil received the nickname “the weekend pill”.
The erection achieved with Tadalafil is usually hard enough for penetration, and sustained for long enough to achieve sexual satisfaction.

In extensive trials in which most men had a history of erectile dysfunction of greater than one year. In addition, there was a medical history of hypertension (29%), diabetes (20%), hyperlipidaemia (16%), coronary artery disease (5%) and depression (4%).
36% were suffering from mile erectile dysfunction, 27% moderate, and 33% severe.
During the trial 3.2% discontinued the treatment due to side effects of taking Tadalafil (compared to 1.3% on placebo) and 1.6% discontinued due to lack of efficacy.
